File of Life



The File of Life card, which is kept in a red plastic pocket labeled FILE OF LIFE, lists your name, emergency medical contacts, medical history, medications, allergies, religion, doctor's name and a health care proxy. The entire pocket is held, with a magnet to the front or side of your refrigerator or in the closest kitchen drawer. The File of Life also contains a small sticker, which can be placed on the back of your front door. This alerts medical personnel that your household has a File of Life.

The File of Life card enables first responders, paramedics, police officers and hospital doctors to obtain a quick medical history when you are unable to offer one. The card also reduces the risk that you may forget to mention something important. As a result, life-saving treatments can begin quicker and be more effective. The File of Life card is not just for elderly or those with a medical history – many in our community are completing a File of Life for all family members, including children that might be left with a babysitter or neighbor that may not have knowledge of that person’s medical condition. The File of
Life is quick, easy, and saves lives.
